The ability of replacing an entire arch of teeth using just four implants is a remarkable phenomenon in dentistry. While attempts of full-arch implants had been made in the 1970s and 1980s, the first ever All-on-4 treatment was completed in 1998 by Dr. Paulo Malo.
Originally, the most common hybrid prosthesis consisted of cast metal substructure with traditional acrylic and denture teeth. However, fast forward to 2022 and there are several material options available. From metal-free options like zirconia, to traditional metal with acrylic. 4 Types of Implant Hybrid Restorations
1. Traditional Acrylic with Titanium Substructure
Frame: Titanium Prosthesis: Acrylic and denture teeth.
- Resiliency reduces impact forces so that it is not transferred to the implant.
- Can be easily repaired, cleaned and polished.
- Acrylic is affordable and lighter
- Acrylic teeth have the longest track record.
- will not wear opposing natural teeth.
- Stains over a period of time.
- Cured acrylic base can result in teeth being dislodged.
- Acrylic does not last as long as zirconia.
- May wear and will chip.
- Wear faster than natural teeth
2. SmartComposite Hybrid Prosthesis
Frame: metal (beryllium free nickel) Prosthesis: milled cobalt-chromium alloy and milled composite.
- Strong, Metal bonds to hybrid composite.
- Highly polishable, it can be repaired in the mouth.
- Wears at about the same rate as natural teeth.
- Less likely to chip compared to acrylic options.
- Lightweight compared to Zirconia.
- Not as durable as full zirconia.
3. ZIRMAX M Hybrid (Monolithic Zirconia Screw Retained Full Arch Prosthesis)
Frame: Zirconia Prosthesis: Zirconia
- Combination of strength and cosmetics.
- Flexural strength of up to 1200 MPa
- Closest feel to natural teeth.
- It has been around for approximately 10 years.
- Most durable option and will be the least likely to chip.
4. Porcelain Fused to Metal Hybrid Prosthesis
Frame: metal (beryllium free nickel)Prosthesis:Porcelain
- Requires the least vertical.
- The most esthetic option.
- Has the most versatility of treatment solutions.
- It is more likely to chip compared to Zirconia but has decades of acceptable longevity.
- Most expensive option.
